It's got to be a tie between either System Shock 2 and Unreal.

SS2 was drenched in this unsettling aura of fear. There was always very limited amounts of ammo, everyone on the ship that you would find was either dead or just about to die, and hell, there was even one section where you were stuck behind a secured see-through door while a massive creature chased two members of the crew and killed them. The best moments of that game would have to be either the long hallway that suddenly swarms with numerous zombies on your return trip back from an objective, the hanging bodies on Deck 3, the man wandering around in the main crew deck imploring you to get help for him (and then subsequently getting killed), or (my favorite) halfway through the game, when you finally go to meet the female doctor who has been guiding you through the ship. You enter her office, and she's lying on a chair dead, then the door locks, the lights go off, and the face of SHODAN illuminates the walls, laughing at you. That freaked me out.

Unreal also had a very suspenseful first level, where you would make your way through the convict transport, and there would be screams of other inmates getting murdered. That was tense.
