This is not to defend them so much, because this stuff SHOULD be tested properly - but as a developer I can tell you sometimes systems react differently under massive scale. There are conditions that sometimes get triggered which you couldn't find with a smaller test base. Of course, that's why many PC MMORPGs have beta user testing - to get that scale. Perhaps that's a route these companies need to consider, because this is becoming too widespread - COD2, COD3 (I was having no issues with Player, but couldn't get into Ranked), even Uno if I recall correctly.
