The sleeve attaches tightly to the sides of the Wii Remote, only adding a few millimeters around the waist. More thickness at the bottom -- perhaps for pounding in that annoying straighten-the-papers WarioWare game -- adds about another centimeter of girth.
But the bulb-shaped top is significantly bigger than the naked Wiimote. Its spongy feel should absorb a lot of energy; I could imagine unintentionally hitting a table or younger sibling without causing injury. I was concerned that the bigger shape would ruin horizontal, NES-style games, but I ended up liking it a little more than the plain Wiimote. My left hand has always felt cramped next to the D-pad, and the Jacket gives it more to hold.
The Jacket also includes a port cover for use without the Nunchuk. I was mildly annoyed by moving the flap to plug in attachments, but gamers could cut that part off of their own Jackets. (See the gallery for a closer view.)
We've shrugged at Nintendo's 18-million dollar move seemingly to preempt American lawsuits. But even minimalist-preferring adult gamers may like the cover's extra size.

...Does anyone here seriously use the wrist strap?

Yes and always. Don't over-estimate yourself thinking that you'd never throw the wiimote. I've had many friends coming over to play and most of them were thinking just like you and avoided the wrist straps. And that's until someone swung the wiimote and had it hit the floor, then flew toward the TV cabinet, and somehow missing all the devices.
After that, everyone put on the wrist straps right away without saying a single word.
Perhaps I am overestimating myself, but I have not had one problem with holding the controller. I just don't see how it can be that difficult...Having to have that strap tightened around my wrist all the time would be quite annoying, too.
I will try it just for sake of better grip on WiiMote.
People throwing controllers around were always here. One guy knew long time ago actually crashed two Sony TV remotes (remotes - not TVs) and was on his third when I met him. The people who have problem controlling their emotions are abundant. WiiMote isn't exception, but just little bit more expensive. Though I doubt they would use jacket, because all of such people are absolutely sure they are able to control themselves perfectly.

Largest WiiMote gripping surface is in fact cover for accus. And on my WiiMote, after Zelda, the cover isn't holding as fast as it was in the beginning.

But since they're free, I ordered four of them anyway. :P
And now, what about those new straps? Where can I get those? Those also better be free.

The last thing I need is to have my girlfriend's dad screaming at me for knocking up his daughter. With a wiimote. To the face.
