I stopped watching/reading bleach a couple years ago, but I'm pretty sure they call Ishida by his first name more often than his family name. In fact I didn't even know his last name was Uryu. That's like getting annoyed if someone refers to the Southpark characters as Kyle, Stan, Kenny, and Cartman. Eric is Cartman's first name, but he would be refferd to as Cartman because that is how the people on the show mostly refer to him as.

Well the dub calls 99% people of the characters by their first name, with a few exceptions like Urahara(they still use his first name sometimes) and Aizen. So I assumed the Stand Operating Procedure in the US was first name basis like the anime dub, especially since these games are dubbed by the same VAs.

That's something I wonder myself. Bleach is quite popular outside of Japan so why not release it at least stateside as well? I'm pretty sure the sales would more than make up for the translation costs in particular if it's on the PS3 (the PSP sadly probably wouldn't work :( )
